There is no definite size of the breast but some times breast become disproportionately enlarged and their excessive weight causes physical symptoms.

Nipple Inversion may be present from birth or from the time of breast development. In this case it is a developmental problem caused by insufficient growth in length of the major milk ducts underneath the nipple. The short ducts pull the nipple inwards causing inversion.these female require breast pump during feeding. If nipple inversion develops in a previously normal nipple. This is an alarming sign,this may be due to inflammation underneath the nipple (periductal mastitis), which eventually causes scarring of the ducts. Breast cancer can also pull on the breast ducts and cause nipple inversion, it is therefore very important to have regular self breast examination.

Nipple eversion

Nipple eversion exercises should be done in developmental problems. Nipple eversion surgery is recommended when exercises do not respond.

Recent onset inversion require mammography and ultrasound, .treatment of underlying cause.This may be a sign of cancer.

Breast pain is sometimes an annoying and quite troublesome symptom, . mostlywoen have some degree of breast pain particularly for a few days prior to each menstrual period. Most breast cancers are ainless in the initial stages. Sometimes this is a transient phenomenon but in some women breast pain is more of a persisting problem. In these cases it can become quite disruptive and can impact on lifestyle. Some women find that severe breast pain wakes them at night when they turn over, or prevents them from exercising. Once the pain gets to this level it is usually worth taking measures to reduce the discomfort.

In addition to hormonal causes for breast pain a small number of women have breast pain caused by inflammation in the milk ducts of the breast.

Breast infections
Breast infections frequently occur during breast feeding and may lead to a breast abscess. The problem is caused by bacteria infecting the milk that is within the breast. Prompt treatment with antibiotics at the outset often resolves the problem. it becomes very painful and abscess develops. The signs of infection are pain within the breast, thickening and hardening of the tissue, and redness of the skin with fever. Abscess needs to be treated by drainage of the pus. This is sometimes possible through a needle under ultrasound control but on occasion a small operation is the most effective and quick way of settling things down.

 

Nonpregnant women may develop infections;.they do require antibiotics,if no response than ultrasound FNAC to be done before surgery,tissue biopsy  is must.

Tumor or mass of the brast are of two types.

   •   Benign [noncancer]
   •   Malignant[cancerous]

   •   Benign [noncancer ]

fibroadenoma is common cause for a lump in young women, often between the ages of 15-30, solid lump may be single or multiple. Fibroadenomas are entirely painless, and  they probably arise due to hormonal changes in the breast. As a consequence, the lump may arise in an area of the breast were there is already more generalised hormonal breast pain and tenderness giving the false impression that the lump is causing the pain. Since they are solid lumps fibroadenomas generally do not disappear, although during pregnancy they may undergo noticeable changes resulting in either their disappearance or their progressive enlargement.

Fibroadenomas are normaly harmless should  be carefully checked and investigated by  ultrasound and needle biopsy. Once a lump has been shown to be definitely a fibroadenoma it is safe to leave under strict supervision if does not disapear surgery is the answer. fibroadenoma is removed by a small  scar operation..we remove all the lumps cosmetically leaving no scar. Breast Cysts

Breast cysts are probably the commonest cause for developing a discreet lump in the breast. Nearly every woman will have cysts in the breast at some time during her life. They are usually very small and women are not aware of these until they have an ultrasound scan. Breast cysts primarily develop as the result of hormonal changes in the breast and are uncommon after the menopause except in women who take hormone replacement treatment (HRT). The most common age to develop breast cysts is between 30-50 years becoming more common the closer one gets to the menopause.

Breast cysts are best diagnosed by an ultrasound scan. develop any serious consequences from breast cysts. They are nearly always entirely harmless and this is very effectively demonstrated by the scan. It is only in very rare cases that breast cysts develop pre-cancerous changes. Most surgeons will advise that breast cysts are left alone. If the cyst is large and uncomfortable a needleasiration  is required.

Cancer

The commonest sign of breast cancer is a lump felt as a separate mass within the breast tissue which is harder than the normal tissue around it. Very often this can be appreciated  by self breast examination .A cancerous lump will be attached to the overlying surface tissue causing either puckering of the skin, which is most noticeable when raising the arms, or inversion of the nipple. Breast cancer can also cause nipple discharge which is frequently blood stained. Breast cancer lumps are practically never painful and generally are felt by accident whilst bathing or during self breast examination. Rarely breast cancer causes a red discoloration of the overlying skin .

There are many types of cancer and there treatment. It is advisable to go to a doctor and discuss individual problem.In our center all modalities of treatment discussed at lenght There are basic investigations required before diagnosing.

Treatment of breast cancer depends on the stage of disease.Surgery is always mendatory   In early cases only small part of breast is removed but bgger tumors whole breast is removed.

We always prefer to advise cosmetic solutions [ reconstruction ].Chemotherapy and radiotherapy  and hormonal therapy is recommended as per pathological diagnosis and stage of disease.

It is necessary for women operated with breast cancer must feel normal after surgery Mastectomy .leaves a flat scar over the chest. Breast reconstruction aims to give a more natural or near natural breast appearance .Many women who now undergo mastectomy opt to have breast reconstructive surgery. This can be carried out at the same time as the mastectomy (immediate reconstruction) or some time later, often after about 12 months (delayed Breastreconstruction) once chemo and radiotherapy is complete. There are two main types of breast reconstruction operation. The simpler types use a silicone prosthesis to create the new breast, while other operation use tissue from other parts of the body such as the back or lower abdomen. Breast Cancer in Men

Although most breast cancers occur in women, about 0.5% - 1% of breast cancers occur in men. 
It is very rare in men under 60. It is also more common in men who have had several close members of their family, male or female, diagnosed with breast cancer
The principles are exactly the same as for women.  The main difference is that most men will need a mastectomy, as there is not enough breast tissue simply to remove the lump alone.

Other treatments, such as hormonal therapy, chemotherapy and radiotherapy will depend on the exact type of cancer and its characteristics as determined by the patient.
